What is Exposure and Response Prevention Therapy (ERP)?

(ERP) is considered the gold-standard treatment for OCD and is the most effective psychological intervention for reducing obsessive thoughts and compulsive behaviors. ERP helps individuals gradually face the situations, thoughts, images, or sensations that trigger anxiety while learning to resist the compulsions, rituals, or reassurance-seeking behaviors that keep OCD going.

Treatment begins with developing a personalized hierarchy of fears, ranging from less distressing situations to more challenging ones. Together, we gradually work through these exposures at a pace that feels manageable while building confidence and resilience along the way.

ERP also focuses on response prevention—the process of reducing and eventually eliminating compulsive behaviors, mental rituals, avoidance, and reassurance-seeking. As clients learn to tolerate uncertainty and anxiety without relying on compulsions, OCD begins to lose its grip.

Depending on your symptoms, exposures may include in vivo exposures (real-life situations) and imaginal exposures, which involve intentionally confronting feared thoughts, scenarios, or uncertainties that cannot be recreated in real life. Through repeated practice, clients learn that anxiety naturally decreases over time and that they are capable of handling discomfort without engaging in compulsions.

The goal of ERP is not to eliminate unwanted thoughts entirely, but to help you respond to them differently so that OCD no longer dictates how you live your life.
